someone with spasticity/spastic muscles/uncontrolled muscle spasms; someone who has cerebral palsy or another specific medical condition. Avoid using "spastic" as a noun or adjective; "spaz" is unacceptable. For the slang usage, substitute: klutzy, clumsy, forgetful, impulsive, reckless. See also disabilities, ditzy.
